Case Management (16+)

Our Case Management program operates using a 6-month service delivery model and works in partnership with individuals to identify areas of strength and determine areas of need. The case manager and individual work together to complete a support plan which is unique to the individual and can assist in addressing areas such as (but not limited to) skills development, issues related to primary health care, employment and access to housing and/or income. The program also assists clients to engage in their local community to reduce their experience of social isolation.

Who is This Program Intended For?

• 16 years of age and above
• Those experiencing serious mental illness to achieve greater independence and improved quality of life
• Individuals willing to work on their recovery goals
