Community Support Coordinator Program (16+)
The Community Support Coordinator Program is funded through the Region of York to provide access to services and supports for individuals residing in York Region Housing. The program offers 1:1 support and referral as well as group based opportunities.
Who is This Program Intended For?
- Individuals 16 years of age and older
- Reside in York Region social tenant housing or are at a risk of housing instability
- Need 1:1 or group based support
The Community Support Coordinator Program Can Support People with Connecting to:
- Housing assistance: eviction prevention, tenant and landlord assistance
- Financial assistance
- Hoarding/clutter/housekeeping
- Mental health services
- Addiction services
- Legal aid
