Community Transitions Team (16+)
The Community Transitions Team is a hospital diversion program, which assists clients with a safe and smooth transition from hospital into the community with the ultimate goal of preventing further hospital visits. We support clients for a 3-month period, helping with housing, developing a wellness plan, support for mental health and substance abuse. We also refer clients to appropriate resources in the community to support their recovery journey.
Who is This Program Intended For?
- 16 and up
- Clients who have recently been discharged from the hospital
