Overview and Philosophy of Care
The Gender Affirming Health Clinic (GAHC) is a free service that aims to create a safe, confidential, and inclusive space across all gender diversities. We provide culturally competent and gender-affirming care to trans and gender diverse people. We work collaboratively with our community partners to ensure supportive and comprehensive care to clients and their families. Our team uses a holistic and compassionate approach when providing services and supports.
Who is This Program Intended For?
- People who identify as trans, gender diverse, non-binary and/or questioning
- 12 years of age or older
- Residents who live in York Region or South Simcoe

The Gender Affirming Health Clinic delivers the following programs and services to eligible clients
- 5-Part Virtual Webinar Series: This webinar is self-guided.
- Trans/Non-binary Monthly Group: Contact the Central Intake team to register.
- Monthly ID Clinic: Available on a walk-in basis.
- Gender Gear Program: Contact the Central Intake team to register.

- Clinical assessment
- Psychosocial supports
- Medical preparative counselling
- Family/caregiver support groups
- Referrals to long-term follow-up supports and specialists
- Access to support from a nurse practitioner
- Psychoeducation about future medical treatment options
- Medical intervention, including prescriptions
- Case management – service navigation and referrals
- Counselling services – exploring gender identity and supporting individuals with their journey
- Physical and sexual health care – health assessments, education and treatment
- Groups and workshops
- Support from a family service worker who provides mental health counseling and case management
- Specialized mental health and psychoeducational support
- Support in navigating conversations about gender, identity, and transition within the family.
- Resources and referrals to affirming peer support groups, community programs, and educational materials.
- Conflict resolution for families struggling with acceptance or communication.
- Guidance on reducing risks of family rejection, housing instability, and youth homelessness.
- Family dynamics assessment
- Parent education sessions
- Parenting consultations
- Parents of youth support group
- Family psychoeducation and/or counselling supports
