Overview and Philosophy of Care
The Gender Affirming Health Clinic (GAHC) is a free service that aims to create a safe, confidential, and inclusive space across all gender diversities. We provide culturally competent and gender-affirming care to trans and gender diverse people. We work collaboratively with our community partners to ensure supportive and comprehensive care to clients and their families. Our team uses a holistic and compassionate approach when providing services and supports.
Who is This Program Intended For?
- People who identify as trans, gender diverse, non-binary and/or questioning
- 16 years of age or older
- Residents who live in York Region or South Simcoe

The Gender Affirming Health Clinic delivers the following programs and services to eligible clients.
Drop-In Services
- 5 part virtual webinar series
- Trans/non-binary monthly group
- Monthly ID Clinic
- Chest binder program
Case Management Services
Our case managers support clients with a variety of needs, including:
- Service navigation
- Referrals to other community services
- Help completing paperwork (e.g., OW forms, etc.)
- Connecting to a family doctor
- Building practical skills to live independently
Counselling Services
Our social workers support clients in exploring all topics related to their gender identity and expression, including:
- Discussing and exploring gender identity
- Transition and resource navigation
- Education
- Collaboration
- Referrals
- Community outreach
Physical and Sexual Health Care
Our nurse practitioners provide primary health care services for clients, including:
- Comprehensive health assessments
- Health promotion/disease prevention
- Medication/treatment management
- Sexual health services
- Referrals
- Transition support
- Hormone Replacement Therapy (including assessment, prescription, monitoring and education)
Groups and Workshops
Clients have the opportunity to participate in our clinical groups and workshops, including:
- AFFIRM CBT
