Psychogeriatric Case Management
The Canadian Mental Health Association – York Region and South Simcoe (CMHA-YRSS) and LOFT’s Integrated Psychogeriatric Outreach Program (IPOP) have a partnership that allows IPOP to identify clients that require Psychogeriatric Case Management support from CMHA-YRSS’ Seniors Case Management program.
IPOP provides a multi-disciplinary team of community clinicians that support older adults who are experiencing a new or functional decline related to mental health or cognitive changes. Through a comprehensive psychogeriatric assessment and access to a consultative appointment with Geriatric Psychiatry, individuals can be identified as requiring a need for ongoing support for up to six months from CMHA-YRSS.
CMHA-YRSS’ Seniors Case Management supports the senior population in reducing barriers such as those related to primary health care, housing and/or income concerns, as well as increasing engagement in their local communities to foster independence, improve quality of life and reduce experiences of social isolation.

Who is this Program Intended For?

Older adults and seniors aged 55 and up living within the communities of York Region and South Simcoe
- Those requiring the support of IPOP and not presently involved with Psychiatry or Geriatric Psychiatry
