STREAM (16+) - Substance Treatment, Recovery, and Empowerment for Addiction Management
Managing Concurrent Disorders and promoting well-being
STREAM is a comprehensive community addictions program that supports individuals who use substances and face mental health challenges. STREAM promotes the client’s well-being by providing addictions counselling and community-based withdrawal management as an alternative to residential detox options.
Services are free, confidential and available in-person or virtually.

Who is This Program Intended For?
- Those 16 years of age or above living in York Region and South Simcoe
- Individuals interested in making a change regarding their substance use and/or addictive behaviours
- Individuals seeking information around harm reduction strategies
- An existing CMHA client involved with either: Assertive Community Treatment Teams, H.O.P.E – Early Psychosis Intervention, Flexible Support, Home First Community Support or Case Management
- Individuals without acute medical complications that would require close observation or treatment in a hospital setting
STREAM Offers
- Assessment and consultation
- Safer substance use and overdose prevention
- Active monitoring and support by a nurse practitioner or addictions counselor
- Education and skills building to prevent relapse
- Linkages to community services
- Group Counselling Support to develop healthier and realistic coping strategies
Program Philosophy
- Integrates Harm Reduction and Recovery principles to support people who use substances and are also facing mental health challenges
- Client & Family Centered: Recovery is a highly individualized process, therefore, the program acknowledges the unique needs, strengths, and aspirations of each individual
- Meet people where they are: Recognizing that individuals have different levels of readiness for change
- Collaboration: Encouraging individuals to take an active role in their recovery process, thereby fostering a sense of control, resilience, and empowerment
